The Opportunity
We are looking for a Registered Nurse (RN) to join our street medicine team, providing mobile medicine services in Los Angeles. Working collaboratively with Providers and Community Heath Workers, the RN will provide comprehensive nursing care, health education, and care coordination for patients in the field. These patients will include people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, and adults with complex medical needs.
Candidates with experience and/or interest in public health, community health nursing, social work or other outpatient experience working with vulnerable populations are particularly well suited for this position. As an RN on our team, you will work collaboratively with the Street Medicine Team to implement care plans, provide direct nursing care, health education, and advocacy for members in need, and participate in case conferences as part of a multidisciplinary team.
What You Do
- Provide direct nursing care to a diverse patient panel with complex health needs in field-based settings where our patients live
- Conduct comprehensive nursing assessments, monitor vital signs, and evaluate patient responses to treatment
- Administer medications, wound care, immunizations, and other nursing interventions per physician orders and nursing protocols
- Provide patient and family education on disease management, medication compliance, and health promotion strategies
- Coordinate care transitions and connect patients with community resources and social services
- Manage a panel of patients under the Enhanced Care Management Program
- Collaborate daily with Providers and Community Health Workers, and support staff through huddles and weekly prioritization meetings
- Document all patient interactions thoroughly to maintain continuity of care across the multidisciplinary team
- Assist with diagnostic procedures and collect specimens as needed
- Support patients in developing health goals and self-management skills to facilitate transition out of homelessness
- Participate in quality improvement initiatives and evidence-based practice implementations
- Other duties as assigned
Who You Are
- Licensed RN in California from an accredited nursing program
- Have strong interpersonal and communication skills to effectively relate to patients, their families, members of the health care team, and the community
- Demonstrated ability to work collaboratively with multidisciplinary teams and support clinical staff
- Skill in organizing workflow to accomplish team objectives while maintaining high-quality patient care
- Ability to work independently, use clinical judgment, and recognize when to escalate concerns to physicians
- Ability to provide culturally competent care to diverse and vulnerable populations
- Ability to work in a dynamic, outdoor environment providing field-based care
- Ability to travel 50-75% - Must have a valid driver's license, automobile insurance and reliable transportation
- Strong assessment skills and ability to adapt nursing interventions to non-traditional care settings
Preferred qualifications:
- Bilingual in English and Spanish strongly preferred
- 1+ years of experience with and comfort working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).
License, certification, and registration requirements:
- Current California Registered Nurse License
- National Provider Identifier
- Basic Life Support (BLS) Certification
- Tuberculosis clearance
Physical Demands: Most work will be in the field working with CalAIM populations (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs). Will likely require substantial walking, standing, sitting in a car, and driving in various weather conditions.
