Through early interventions designed around social determinants of health (SDoH), world-class care focused on chronic disease, and human-centered technology, we believe we can build a healthcare model that allows for all patients to live their fullest lives. We are a fast-growing, impact-focused, Y Combinator company created out of the University of Southern California’s D-Health Lab with the idea that empowering government, healthcare, and nonprofit services with population-based data could help usher in a new era of preventive public health. Today we are building a full stack medical network that leverages our predictive capabilities to provide a frictionless experience for both our patients and care providers.

The Opportunity

In collaboration with the Independent Drivers Guild (IDG) and Workers Benefit Fund (WBF), Akido is launching an innovative health program in New York City designed specifically for independent drivers (e.g., Uber, Lyft, and delivery workers).

We’re seeking a Comprehensive Care Coordinator who is passionate about health equity, thrives in a dynamic environment, and wants to help build a new model of community-centered care from the ground up.

This is not a traditional administrative role — it’s a hands-on, relationship-driven position that blends care coordination, program operations, and community engagement. You’ll work closely with our multidisciplinary care team (including MDs, NPs, RDs, and Health Coaches) to keep the program running smoothly, ensure patients get the support they need, and help refine systems that make healthcare more accessible and human.

What you’ll do

Program Coordination

  • Manage scheduling for patient appointments (virtual and in-person) to optimize access and reduce no-shows.
  • Conduct outreach to patients via calls, texts, or in-person engagement to confirm appointments, check in on progress, and encourage participation.
  • Support onboarding by coordinating consent forms, intake paperwork, and interpreter services.
  • Help patients navigate insurance, IDG benefits, or community resources to ensure they can access needed care and services.
  • Maintain accurate documentation in the EMR, including patient interactions, outcomes, and follow-up needs.
  • Communicate across the care team (MD, NP, RD, HC) to ensure timely, integrated, and coordinated care delivery.

Program Logistics & Operations

  • Plan, implement, and manage program logistics (e.g., group sessions, workshops, community events).
  • Coordinate logistics such as space setup, attendance tracking, and post-session follow-ups.
  • Track key program metrics and maintain dashboards or reports to support quality improvement and performance monitoring.
  • Provide operational support to providers by handling non-clinical tasks (e.g., patient messaging, reminders, referrals) to allow them to focus on clinical care.
  • Support continuous process improvement — gather feedback from patients and staff, identify workflow challenges, and help design better systems.

Community Engagement & Outreach

  • Serve as a trusted representative of the program in the community — building relationships with drivers, families, and local organizations.
  • Participate in enrollment activations, outreach events, and health fairs across NYC’s five boroughs.
  • Contribute to outreach campaigns that promote preventive care and increase program visibility.

  • Excited to build systems and workflows that make healthcare delivery more efficient and patient-centered.

Required Qualifications

  • 3+ years of experience in care coordination, program coordination, case management, or community health.
  • Demonstrated ability to work with diverse populations and communicate cross-culturally.
  • Experience with EMRs or health documentation systems.
  • Strong organizational and multitasking skills; ability to manage competing priorities.
  • Comfort working in field-based, mobile, or virtual environments.

Preferred Qualifications

  • Background in community health, public health, or health program administration.
  • Experience with chronic disease management programs or wellness coaching.
  • Bilingual (Spanish, Bengali, French, Haitian Creole, or Arabic preferred).
  • Familiarity with NYC community resources and benefits navigation.

Work Environment

This is a mobile and hybrid role with frequent travel throughout NYC’s five boroughs. You’ll work in a variety of settings, including mobile clinics, community partner sites, and virtual platforms. Flexibility is key, as hours may occasionally include early mornings, evenings, or weekends to best reach our patient population.
