Summary

To enable Curana Health’s expansion and performance, the Population Health team is seeking a high-performing leader to provide clinical guidance and oversight to our value-based care delivery model and contracts. This role will report to the President - Medical Group and provide oversight and guidance of: care model delivery development; provider clinical care pathways; educating our provider team on Curana model; in addition to acting as a clinical representative for the Curana population health team in external forums. Key Responsibilities

• Partner with the President - Medical Group, Chief Population Health Officer, and VP, Clinical Programs to develop and maintain the overarching clinical model that Curana will use to support value-based care delivery to our patients including interdisciplinary team structure, systems/tools, and processes.
• Develop clinical pathways for key conditions/events common to caring for patients in senior living environments.
• Provide oversight of clinical documentation improvement efforts.
• Oversee creation of clinical content and sequencing/prioritization of content to educate providers on clinical model.
• Lead clinical quality committee.
• Represent Curana in meetings with external partners including facilities, Medicare Advantage plans, CMS and industry associations.
• Live the clinical model by spending a small portion of time on direct patient care

Background & Requirements

• MD or DO degree
• Valid, unencumbered license to practice medicine in 1 or more states
• 7+ years of experience in clinical leadership roles in value-based care settings/programs
• Familiarity and experience with value-based care concepts and payment models, e.g. ACOs, Medicare Advantage, capitation, shared savings
• Excellent organizational and time management skills
• Strong verbal and written communication skills with experience in conducting executive-facing discussions.
• Comfort with ambiguity and ability to shift focus across priorities as business needs evolve.
• Proficiency in business systems including Microsoft Office, customer relationship and project management tools
