  • Compensation: $600 per 12 hour Saturday or Sunday daytime shift
  • Full-time benefits eligible if able to commit to 5+ shifts per month.
  • Fully remote role, but must have an active North Carolina license, with additional licenses strongly preferred.

SUMMARY

The After Hours Call Team is responsible for managing the continuity of care for residents in senior living facilities overnight and on the weekends. Members of this team respond to calls from post-acute care facility staff regarding patient care, evaluation, treatment of conditions, medication, and orders for Curana Health patients. The work is performed remotely via telephone and telemedicine with the goal of assessment and evaluation of options for treatment in place and/or transfer out to an acute care facility. The After Hours Call Team coordinates care and shares documentation in the EMR with the local, day-time Curana providers.

RESPONSIBILITIES

  • Respond to telephone calls from post-acute facility staff using advanced call center technology.
  • Provide ongoing telephonic evaluation and treatment of chronic, co-morbid illnesses, as medically appropriate.
  • Remotely evaluate/treat residents for changes in condition.
  • Review medications, orders, and diagnostic tests, and respond to clinical abnormalities appropriately for Curana Health patients. Submit new orders, when necessary.
  • Review existing patient record and document triage notes for local, day-time Curana provider to review the next business day.
  • Coordinate care with collaborating physicians as indicated for overall medical management.
  • Awake and logged into clinical and IT systems to be ready to quickly answer incoming calls and review relevant medical history.
  • Other duties as assigned

EXPERIENCE

  • 2+ years of demonstrated experience in telemedicine and/or telephone call triage
  • 2+ years of demonstrated experience caring for patients in post-acute care facilities (ex: Skilled Nursing Facilities, Assisted Living, Independent Living, Memory Care)

SKILLS AND ABILITIES

  • Strong clinical decision making skills
  • Knowledge of clinical best practices for geriatric patients
  • Must possess excellent written and oral communication skills
  • Must possess traits of punctuality, organizational skills, and ability to function within a team

EDUCATION

  • Valid nurse practitioner or physician assistant license in state of operations & current board certification
  • Multiple state licenses preferred for cross coverage
