Job Description:

The Director of Clinical Operations for Palliative & Supportive Care is a key leadership position responsible for overseeing and managing the operations for our palliative care program. This position is responsible for developing and implementing the company’s overall business strategy and growth plan for palliative care services in post-acute and senior living facilities. This role requires a proven history of leading a team of providers with attention to clinical best practice, outcomes measurement, growth and facility engagement. The Director is expected to foster positive and effective working relations with senior leadership, local market management, providers, senior living facilities and business ancillary departments for the purpose of achieving the company’s vision, mission and objectives.

Responsibilities:

  • Provide strategic leadership and direction for palliative care program expansion
  • Manage a team of Palliative Care Specialized Advanced Practice Providers (APPs)
  • Monitor and analyze quality metrics to assess the effectiveness and efficiency of care delivery, identifying areas for improvement and implementing quality improvement initiatives.
  • Serve as a subject matter expert and national voice for Curana Health on topics related to palliative care, end of life and hospice
  • Develop and deliver Palliative Care education internally and externally
  • Collaborate with the leaders of the population health and care management departments to ensure aligned priorities and efficient use of shared resources
  • Partner with market directors to expand palliative care services into new and existing facilities
  • Build and maintain relationships with senior living facilities, collaborating with their staff to optimize referrals and meet the unique healthcare needs of residents
  • Recruit, hire orient, mentor and train new staff to the Palliative Care Team
  • Actively participate in company committees or task forces related to clinical operations, quality improvement, collaboration/supervision and patient safety, providing expertise and guidance.
  • Foster a culture of patient-centered care, promoting a positive and compassionate environment for patients, families, and staff.
  • Ensure compliance with relevant healthcare regulations, accreditation standards, and patient safety guidelines.
  • Stay updated with the latest developments and trends in palliative care, geriatric medicine, and senior living, incorporating evidence-based practices into clinical operations.
  • Serve as a liaison with external stakeholders, including physicians, healthcare organizations, community resources, and regulatory bodies.
  • Other duties as assigned.

Experience:

  • 5+ years demonstrated experience leading palliative care programs
  • Advanced Practice Practitioner preferred

Skills and Abilities

  • In depth knowledge of palliative care evidence-based guidelines
  • Ability to organize and engage multiple stakeholders to provide best in class palliative care services
  • Strong relationship building & business development skills
  • Excellent communication, interpersonal, and team-building skills.
  • Strong analytical and problem-solving abilities, with a focus on data-driven decision-making

Education:

  • Master's degree in health-related field or nursing preferred

Travel Requirements:

  • Remote role
  • Up to 20% travel required in order to visit providers and facilities
