Position Name: Director, Travel HRA Team
Position Type: Full-Time
Location: Various long-term care facilities (SNFs, Assisted Living, Independent Living) within the assigned region; some travel required.
Reports To: CMO Population Health
Job Summary: The Director Travel HRA Team will oversee and supervise a traveling team of Nurse Practitioners (NPs) conducting Health Risk Assessments (HRAs) within long-term care settings, including skilled nursing facilities (SNFs), assisted living facilities (ALFs), and independent living (IL) communities. This role focuses on clinical oversight, performance management, operational coordination, and ensuring high-quality patient care. The Director will collaborate with leadership to strategically deploy team members and maintain smooth operations across facilities. The Director will also be responsible for providing training and education on Risk Adjustment and accurate documentation to a broader set of Curana providers.
Responsibilities include:
1. Supervisory and Clinical Oversight:
- Providing guidance, mentorship, and clinical oversight.
_Ensure the compliant documentation to highest accuracy and completeness possible
- Ensure clinical quality standards are met through regular performance monitoring and review of HRA outcomes.
- Provide case-specific clinical support and decision-making when needed.
2. Deployment and Resource Allocation:
- Use data and leadership input to determine strategic deployment of NPs across facilities based on patient and organizational needs.
- Monitor workload distribution and adjust schedules or assignments to ensure optimal coverage and efficiency.
3. Training and Performance Monitoring:
- Develop and implement training programs to ensure NPs are skilled, confident, and up-to-date with clinical guidelines and best practices.
- Conduct regular performance evaluations and provide constructive feedback to team members.
- Identify areas for improvement and facilitate continuing education opportunities as needed.
4. Operations and Coordination:
- Ensure smooth operations by collaborating with facility leadership and internal teams.
- Address logistical challenges, resolve conflicts, and maintain strong communication channels between all stakeholders.
- Oversee compliance with organizational policies, regulatory standards, and documentation requirements.
5. Quality Assurance and Process Improvement:
- Conduct regular audits of clinical performance, patient records, and assessment outcomes to identify gaps or areas for improvement.
- Lead initiatives to enhance the quality, efficiency, and consistency of HRAs within the program.
- Report on program performance metrics to leadership and provide actionable insights for improvement.
- Participate in provider education group and individual sessions for Curana providers outside of travel team.

Qualifications:
- Nurse Practitioner (NP) degree from an accredited institution.
- Board certification in Family Medicine, Internal Medicine, Geriatrics, preferred
- Active and unrestricted medical license in two or more states where Curana operates, and wiliness and ability to be licensed in multiple states.
- Experience in risk adjustment, particularly within an HRA vendor or an organization managing Medicare Advantage risk, is highly desirable.
- Experience working in long-term care or geriatrics is highly desirable.
- Proficiency in electronic medical record (EMR) systems.
- Strong organizational, leadership, and communication skills.
