Summary
The Care Navigator supports Curana CCM Care Managers and Curana providers with non-clinical tasks. The ideal candidate possesses a strong background in medical administration, excellent communication skills and the ability to adapt to virtual platforms.
Essential Duties & Responsibilities
Patient Support
- Handle inbound calls and requests from facilities / facility staff
- Address patient and durable power of attorney (DPOA) inquiries via telephone
- Respond to patient or caregiver messages received via the Curana Patient Portal
- Assist patients with scheduling follow-up appointments with Curana Providers or specialists
Provider / CCM Support
- Manage electronic health records (HER) and ensure accurate and up-to-date patient records
- Assist with patient census management activities
- Maintain patient rosters for patients enrolled in Chronic Care Management
- Upload new residents into the Care Management platform
- Update flags and census information in GehriMed
- Coordinate documents needed for review or signature by a provider
- Facilitate provider orders and escalate findings
- Obtain patient records and diagnostic test results
Communication Support
- Answer and manage incoming calls professionally and courteously
- Collaborate with the Curana Care Team to ensure seamless communication within our health network
Other duties as assigned
Qualifications
Required Education and Experience
- High school diploma or equivalent
- 1+ years of experience working in a medical office , Senior Living Community engagement or other related fields
- 1+ years experience in Electronic Health Records (EHR) documentation or other practice management tools
Required Skills
- Experience with computers
- Office 365 Suite
Travel Requirements:
- 100% remote position requiring a reliable high-speed internet connection.
