Summary

We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality, facility-based care within an Inpatient Rehabilitation Facility (IRF).

In this role, you will deliver direct patient care for medically complex patients recovering from acute illness, injury, or surgery. You will collaborate closely with physicians, therapists, nursing staff, and care teams to optimize medical management, support functional progress, and reduce avoidable hospital readmissions.

Essential Duties & Responsibilities

Patient Care & Clinical Management

  • Perform comprehensive evaluations, including admission history and physicals, and ongoing reassessments

  • Manage acute and chronic medical conditions in a post-acute, inpatient rehab setting

  • Evaluate and treat changes in patient condition, escalating care as appropriate

  • Review labs, imaging, and diagnostics; adjust treatment plans accordingly

  • Provide episodic and interventional care visits as needed

Rehabilitation-Focused Care

  • Support patients recovering from stroke, orthopedic procedures, cardiac events, and other complex conditions

  • Collaborate with physical, occupational, and speech therapists to align medical care with functional goals

  • Optimize patients’ medical status to enable participation in therapy and maximize recovery outcomes

Care Coordination & Collaboration

  • Partner with collaborating physicians for overall medical management

  • Work closely with interdisciplinary teams including therapists, nursing, case management, and social work

  • Participate in care planning meetings and interdisciplinary team conferences

  • Coordinate discharge planning to ensure safe transitions to home or next level of care

Patient & Family Engagement

  • Communicate regularly with patients and families regarding care plans, progress, and expectations

  • Participate in family conferences when appropriate

  • Provide education on disease management, medications, and recovery expectations

Documentation & Compliance

  • Maintain accurate, timely documentation in the EMR in accordance with regulatory and facility requirements

  • Ensure compliance with IRF, CMS, and organizational standards

  • Support coding accuracy and medical necessity documentation

Quality & Outcomes

    • Contribute to quality improvement initiatives focused on reducing hospital readmissions and improving functional outcomes

    • Support performance metrics tied to patient outcomes, length of stay, and satisfaction

    • Assist in identifying opportunities to improve care delivery within the facility

Full-time employees are eligible for Curana Health’s full benefits package. Part-time employees may be eligible for limited benefits as applicable.

Qualifications

  • Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
  • Masters Degree in Nursing or Physician Assistant Studies
  • Strong leadership, interpersonal, and problem-solving skills preferred.
  • Must possess excellent written and oral communication skills
  • Must possess traits of punctuality, organizational skills, and ability to function within a team.

This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement. From that time forward, please be advised that Periodic motor vehicle (MVR) checks may be conducted as a condition of, and/or for, continued employment.
