Summary
We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our PRN wound care rounding team. In this role, you will provide specialized wound assessment and management services across skilled nursing and senior living facilities, working on a flexible, as-needed basis. This position offers a rewarding opportunity to make a meaningful impact on the health and quality of life of seniors through expert wound care intervention and collaboration with facility teams.
Essential Duties & Responsibilities
- Wound Assessment and Management: Perform comprehensive wound assessments, develop individualized wound care treatment plans, and manage wounds including pressure injuries, surgical wounds, diabetic ulcers, and venous/arterial wounds.
- Rounding and Monitoring: Conduct scheduled and unscheduled wound care rounds across assigned facilities, monitor healing progress, reassess treatment plans, and escalate complex cases as appropriate.
- Care Coordination: Collaborate with facility nursing staff, primary care providers, and other members of the care team to ensure seamless, coordinated wound care management tailored to each resident's needs.
- Education and Training: Educate facility staff, residents, and families on wound prevention strategies, appropriate wound care techniques, and signs of infection or deterioration to support optimal outcomes.
- Documentation and Compliance: Maintain accurate, timely, and thorough wound care documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols.
- Quality Assurance: Participate in quality improvement initiatives to enhance wound care outcomes, reduce facility-acquired pressure injuries, and support regulatory survey readiness.

Qualifications
- Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
- Master's degree in Nursing or Physician Assistant Studies.
- Experience in wound care, long-term care, or post-acute settings strongly preferred.
- Wound care certification (e.g., CWS, CWCN, WCC) preferred but not required.
- Strong clinical assessment, interpersonal, and problem-solving skills.
- Excellent written and oral communication skills.
- Punctuality, organizational ability, and capacity to work independently across multiple facility settings.
This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement.
