Job Description
A bit about this role:
This position represents an exceptional opportunity for an enthusiastic physician leader to drive the strategic vision and clinical excellence of Devoted Medical's Transitions of Care (TOC) Program. The TOC program provides virtual care in the 30-day window after a patient is discharged from the acute setting (hospital or skilled nursing facility), serving patients across multiple states with the goal of preventing readmissions through exceptional, comprehensive wrap-around care. The program leverages cutting-edge technology and artificial intelligence (AI) to provide the best care possible.
The Medical Director will provide clinical leadership for a large, interdisciplinary team of healthcare professionals, including nurse practitioners, registered nurses, medical assistants, and clinical coordinators. This role combines direct patient care (approximately 20-40% of time) with strategic program leadership, team development, and business partnership. The Medical Director is responsible for clinical outcomes, including readmission rates, while working closely with finance and operations teams to ensure the program delivers on key business objectives during this exciting growth phase. Because Devoted is a tech-forward, fast-moving organization, the Medical Director will assist in the implementation of AI tools that enables an unprecedented quality of care for our patients.
The position requires an entrepreneurial physician who can translate senior leadership vision into actionable strategies, lead complex quality improvement initiatives, foster a culture of clinical excellence across a geographically distributed team, and drive adoption of an ever-growing array of AI tools. The Medical Director serves as the final clinical escalation point and provides oversight through collaborative practice agreements across multiple states.
Your Responsibilities and Impact will include:
Clinical Leadership & Patient Care:
Provide direct virtual patient care approximately 20-40% of time, focusing on complex transitions of care cases
Serve as the final escalation point for clinical questions and complex patient management decisions
Oversee clinical outcomes for the entire TOC program, with primary accountability for readmission rates
Supervise nurse practitioners (NP) through collaborative practice agreements across multiple states
Lead weekly interdisciplinary rounds (3 hours/week) and readmission conferences to review high-risk patients
Team Management & Development:
Responsible for recruitment, onboarding, and clinical development of a new cadre of specialized providers delivering advanced virtual care.
Provide clinical oversight and strategic direction for a team of professionals across multiple disciplines
Directly manage a growing provider workforce through NP managers who supervise front-line clinical staff
Collaborate closely with operations leads and senior managers for nursing, medical assistants, and coordinators
Foster team culture and facilitate safe, open communication across all team roles and geographic locations
Program Development & Strategy:
Develop and implement clinical protocols, policies, and care pathways for the TOC program
Lead quality improvement pilots and initiatives to enhance patient outcomes and team efficiency
Translate senior leadership vision and strategic initiatives into operational plans for front-line teams
Drive program growth strategy and expansion initiatives in collaboration with executive leadership
Drive adoption of new technologies (with an emphasis on artificial intelligence enablement) and workflows to improve team efficiency and patient outcomes
Lead change management initiatives to ensure smooth implementation of new processes and systems
Business Partnership & Operations:
Partner closely with finance team to maintain program budget and deliver on business outcomes
Analyze program metrics and implement data-driven improvements to clinical and operational performance
Collaborate with interdisciplinary leadership team to ensure alignment across all program functions
Provide regular reporting and updates to Chief Medical Officer on program performance and strategic initiatives
Technology & Innovation:
Utilize and provide feedback on Devoted's proprietary electronic health information system (Orinoco)
Contribute to the development of AI-enabled clinical tools to support care within the TOC service.
Required skills and experience:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
Board certification in Internal Medicine or related specialty
Minimum 3 years of post-residency clinical experience
Strong background in inpatient care and understanding of transitions of care period
Experience in management of chronic medical diseases including heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections
Leadership or management experience with demonstrated ability to lead teams through organizational change
Excellent communication skills with ability to translate vision and strategy to front-line teams
Strong analytical and problem-solving abilities with experience using data to drive clinical and operational improvements
Collaborative mindset with ability to work effectively with interdisciplinary teams and business partners
Desired skills and experience:
Experience in hospital medicine, post-acute care, primary care, or geriatrics
Previous experience as a medical director or in similar physician leadership role
Experience with telehealth platforms and virtual care delivery models
Background in quality improvement methodologies and leading clinical initiatives
Experience working in high-growth healthcare organizations
Knowledge of healthcare regulatory requirements and compliance standards
Entrepreneurial, growth-minded, and adaptable team player who is ready to innovate on transitional care delivery models
Licensure and Certification:
Active medical license in good standing, with ability and willingness to obtain licensure in additional states as program expands
Board certification in Internal Medicine or related specialty
Active BLS certification at time of hire and must maintain while employed at Devoted Medical
DEA registration in good standing
