A bit about this role:

Devoted Medical has built a best-in-class virtual Diabetes Clinic to manage and coordinate care in a manner consistent with Devoted’s values of treating every member like family. Fundamentally, we want to help members navigate the healthcare system in a better and safer way, and we want to empower members to change the trajectory of their chronic diseases for the better. Disease management is critical to achieving this for our members with diabetes. Our multidisciplinary team combines remote monitoring, medication management, and high-touch disease education to deliver seamless speciality care in the comfort of our members’ homes.

As the Diabetes Clinical Guide Manager, you will have the opportunity to directly mentor and lead our team of Certified Diabetes Care and Education Specialist (CDCES) clinical guides within the Diabetes Clinic. The Diabetes Clinical Guide Manager will report to the Senior Manager of the Diabetes Clinic.

Required skills and experience:

  • 3+ years of experience leading frontline clinical teams with accountability for team-level outcomes

Desired skills and experience:

  • Demonstrated ability to champion new initiatives and achieve direct business outcomes

  • You have great clinical and non-clinical judgment and are able to coach around performance to drive clinical effectiveness

Your Responsibilities and Impact will include:

Serve as direct manager of a team of Diabetes clinical guides (CDCES): (50%)

  • Act as a clinical champion for the work - you are a player-coach, modeling excellent care delivery for the clinical guides

  • Perform goal setting and coaching of direct reports to achieve business goals and clinical performance outcomes as set by clinic leadership; this includes monitoring the clinical guides’ results as shown on individual performance and productivity dashboards that are aligned with the patient journey map and desired clinical outcomes

  • Complete regular reviews of direct reports’ work and hold clinical guides accountable to individual performance through audits, shadowing, and 1:1 meetings; this includes creating performance improvement plans, when needed

  • Perform regular clinical audits (QA) that include a mix of note review and listening tours

  • Steward our member-centric and open clinical culture to our members and colleagues

  • Recruit, interview, hire, and onboard new clinical staff

  • Identify needs, problem solve, and solution with the leadership team to achieve improvements and increased standardization of the clinical workflow

  • Create workflows, assessments, and plans of care that create efficiencies for the team and show outcomes for member

  • Assist the team in working with escalated member needs

  • Coordinate and facilitate ongoing education to existing staff (in addition to owning the onboarding experience for new teammates)

  • Meet with your manager regularly to discuss team performance, successes, and areas for improvement

Direct clinical care: (50%)

  • Maintain an active panel of members enrolled in the Diabetes Clinic. Maintaining a panel is essential to stay immersed in the workflows and maintain expertise in the care model. Maintaining a panel includes, but is not limited to the following: Screen members for Diabetes Clinic eligibility and appropriateness; Complete detailed assessments and continue following up with members in the Diabetes Clinic to complete ongoing education on all pillars of diabetes management (disease process, medications, glucose monitoring, lifestyle counseling, contingency planning) ; Work with the members to create and achieve collaborative SMART goals; Utilize motivational interviewing skills with the goal to influence positive sustained behavioral changes; Identify gaps in care and provide resources; Collaborate with other Diabetes Clinic team members (nurse practitioners, social workers, pharmacists) to develop plans of care and optimize medication regimens to help members achieve goals; Maintain documentation related to member and provider interactions

  • Continue to improve skill set with participation in education series within the Diabetes Clinic and outside providers of continuing education

Salary Range: $75,000.00 - $103,000.00 annually

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
