A bit about this role:

  • The Behavioral Health Care Manager plays a vital role as a core member of the BH team, that includes the patient’s primary care provider, a psychiatric consultant, and other clinical staff.

  • This team is embedded in Primary care to deliver integrated, patient-centered care within the on-line devoted health system.

  • This position is a remote role responsible for supporting and coordinating the mental and physical health care needs of patients on an assigned caseload and from internal PCP referrals.

  • Working within the Collaborative care model among other models of care, the Behavioral Health Care Manager ensures patients receive comprehensive, compassionate, and effective care.

Responsibilities and Impact will include:

  • The BHCM bridges the gap between mental and physical health by coordinating care, tracking progress, and delivering care to empower patients on your caseload to stay on track—making collaboration seamless and outcomes stronger. You will do this by:

  • Screening and assessing patients for common mental health and substance abuse disorders and facilitating patient engagement and follow-up care.

  • Providing brief behavioral interventions using evidence-based techniques such as behavioral activation, CBT, problem-solving treatment, motivational interviewing, or other treatments as appropriate; patient education about common mental health and substance abuse disorders and the available treatment options

  • Tracking patient follow up and clinical outcomes using a registry. Documenting video and telephone encounters in the registry and EHR and using the system to identify and re-engage patients. You will use the registry and regular patient follow-up to track treatment response and other important clinical outcomes to enable early recognition and treatment adjustments, making remission of symptoms more likely.

  • Participating in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicating resulting treatment recommendations to the patient’s medical provider. Consultations will focus on presentation of patients new to the caseload and those who are not improving as expected under the current treatment plan.

  • Supporting psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.

  • You may facilitate co-ordination of care to available resources as needed and provide social work case management including bio-psycho-social assessment to identify and support needs for PCP members.


Required skills and experience:

  • Education:

    • Licensed Clinical Social worker, Licensed Professional counselor or Licensed in a related field of study

    • Completion of degree and all post-degree supervision for licensure and at least 2 years of post-degree experience.

  • Experience/ Skills

    • Experience of successfully delivering evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).

    • Ability to effectively engage patients in a relatively short-term therapeutic relationship, via video and/or audio.

    • Evidence of high degree of organizational, collaborative and communication skills

  • Licenses:

    • Licensed in one or more states. Willingness, education and experience sufficient to obtain multiple state licenses.

Desired skills and experience:

  • Prior experience of CoCM and/ or willingness to acquire new skill sets in this innovative model of care.

Salary Range: $70,000-$80,000 / year

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
