Responsibilities

The Licensed Professional Nurse/Licensed Practical Nurse provides intermittent skilled nursing services to patients in the home setting according to the patient’s plan of care. As a Licensed Professional Nurse/Licensed Practical Nurse you will:

  • Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities, including activities of daily living as outlined in the patient Plan of Care
  • Submit completed skilled nursing visit notes as requested by Clinical Supervisor
  • Participate in discharge planning process per Medicare Criteria
  • Attend staff meetings, team conferences and educational in-services as required
  • Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Qualifications

Qualifications

  • A current unencumbered State Professional Nurse License
  • Active CPR Certification
  • Must maintain a valid driver's license, reliable transportation, and good driving record
  • The ability to make sound professional clinical judgment
  • The ability to assess and document patient needs and meet those needs
  • Proficiency in Microsoft Office
