The Licensed Practical Nurse provides intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. As a Licensed Practical Nurse you will:

• Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care
• Submit completed skilled nursing visit notes and participates in the discharge planning process per Medicare Criteria
• Provide assistance with activities of daily living (ADL’s) which may include personal hygiene, changing of bed linens, dressing and ambulation and transfers as the patient need arises
• Attend staff meetings, team conferences and educational in-services, as needed
• Participate in Process Improvement (PI) program by assisting with collection of data
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Qualifications

• A current unencumbered State Professional Nurse License
• Active CPR Certification
• Must maintain a valid driver's license, reliable automobile, and good driving record
• Proficiency in clinical skills
• Proficiency in Microsoft Office

Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
