Responsibilities

The Nurse Practitioner, Clinical Triage for the Call Us First team will act as the primary provider and clinical point of contact for patients that require evaluation for issues that may require further clinical intervention. The position will act as a liaison between triage teams and local clinical teams.

Essential Duties and Responsibilities

  • Act as clinical resource for clinical triage teams for patients that require additional clinical evaluation
  • Evaluate and treat patients via telehealth to address acute needs
  • Act as a liaison between central triage resource and local clinical teams by providing updates on patients and coordinating follow up visits to provide comprehensive care
  • Address time sensitive medication refills for patients
  • Address time sensitive lab results and other results for patients while coordinating care with local teams
  • Interact with Primary care provider of record for patients that require further escalation or evaluation in Emergency Room
  • Address time sensitive task messages for patients requiring additional clinical support
  • Assist in service recovery for patients that require timely care
  • Working closely with pharmacy team for comprehensive patient care
  • Sign home health, DME, death certificates, and other orders as needed
  • Frequently review Quality metrics, pod level performance and identify areas for improvement leading to superior Quality (HEDIS) performance
  • Educate patients on the Urgent Care Triage program, to help reduce hospital readmission, improve patient outcomes and patient satisfaction
  • Identify any gaps in care and assist in ordering/coordinating with external resources. Example: home health, hospice and other ancillary services as needed
  • Work collaboratively with RCMO and Medical Director to identify opportunities for care improvement
  • Additional duties as assigned by the Regional CMO and/or medical director as related to clinical pod performance and outcomes

Qualifications

Required Knowledge, Skills and Experience

  • Must be a certified Nurse Practitioner with active licenses in coverage states
  • Must have active DEA license
  • Excellent written and verbal communication skill
  • Ability to react decisively and quickly in urgent and emergent situations
  • Demonstrated experience with multi-tasking
  • Strong communication skills (written & spoken)
  • Strong organizational skills and attention to detail

Preferred Knowledge, Skills and Experience

  • 1 year of experience at HarmonyCares Medical Group
  • Experience with APRIMA EMR
  • Experience with telehealth and coordinating care virtually.
  • Multistate License (MSL)
