THE OPPORTUNITY:
The Quality Nurse will play a key role in supporting the implementation of hypertension quality improvement (QI) efforts across Chicago as part of the Live Healthy Chicago initiative. This nurse will work as part of a multidisciplinary Quality Improvement Team, including a pharmacist and medical director—to strengthen hypertension screening, management, and follow-up in both clinical and community-based settings.
The ideal candidate is passionate about improving health equity, skilled in clinical quality improvement, and experienced in collaborating with FQHCs and community partners.
ABOUT THE INITIATIVE:
Wellness West is supporting the Live Healthy Chicago (LHC) initiative which is a citywide collaboration designed to improve hypertension management and cardiovascular health outcomes across Chicago’s communities. Through partnerships with community-based organizations (CBOs) and federally qualified health centers (FQHCs), LHC aims to reduce disparities in hypertension and related chronic diseases by meeting patients where they are, within their neighborhoods and trusted community settings.

WHAT YOU CAN LOOK FORWARD TO:
- Serve on LHC Quality Improvement (QI) team supporting collaborative wide blood pressure/cardiovascular health improvement initiatives.
- Lead workflow development and roll out plan to support quality improvement activities such as accurate blood pressure measurement, documentation, and follow-up processes in community and clinical settings.
- Collaborate with medical home sites to train and mentor nurses, medical assistants, and other clinical staff on proper blood pressure measurement techniques and hypertension management best practices.
- Provide onboarding and ongoing support to med home QI teams and staff conducting hypertension screenings in community settings and partner clinics.
- Collaborate with FQHCs and community-based organizations to strengthen care coordination and patient engagement in hypertension management.
- In collaboration with the QI team, conduct quality improvement projects, including data review, gap analysis, and development of improvement plans to enhance hypertension control rates.
- Support mobile and community-based screening events, ensuring adherence to evidence-based screening protocols and appropriate referral to care.
- Work closely with the pharmacist and medical director to develop and implement hypertension management strategies aligned with initiative goals.
- Participate in continuous quality assurance and evaluation processes to monitor outcomes and share best practices across participating sites.

WHAT YOU’LL NEED TO SUCCEED:
- Active Registered Nurse (RN) license in the State of Illinois.
- Minimum 6+ years of nursing experience, preferably in primary care, public health, or community health settings.
- Experience working with federally qualified health centers (FQHCs) or similar safety-net providers.
- Demonstrated knowledge of hypertension, heart disease, and stroke prevention and management.
- Strong understanding of clinical quality improvement and performance measurement principles.
- Demonstrated skills in program development, training, and education.
- Excellent communication, facilitation, and training skills.
- Ability to work collaboratively across multidisciplinary teams and with diverse community partners.
- Valid driver’s license and willingness to travel across Chicago for site visits and community events.
- Experience with community-based health initiatives or population health programs.
- Familiarity with quality improvement methodologies (e.g., PDSA cycles, Lean, Six Sigma).
- Prior work in value-based care or population health environments.
