THE OPPORTUNITY:
Reporting to the Chief Medical Officer, the Associate Medical Director drives MHN’s clinical innovation, care delivery redesign, and value-based performance across markets. This role strengthens care design to improve outcomes, reduce total cost of care, and support strategic growth within Medicare, Medicaid, and ACO models. Success in this role requires frequent travel to provider sites, fostering collaboration, coaching and guiding clinical teams, influencing workflows, and enhancing the patient and provider experience. The individual must be based in Chicago, IL.

  • Hybrid schedule: work onsite at MHN headquarters on Tuesday and Wednesday each week, with regular onsite visits to partners in the Chicago area and travel 1–2 times per month to partners outside the Chicago area.

WHAT YOU CAN LOOK FORWARD TO:

    Care Model Innovation & Advancement:
  • Lead the design and execution of care models that align with MHN’s clinical and operational goals.
  • Collaborate on clinical strategies and interventions that improve outcomes and care delivery
  • Partner with the CMO and Medical Directors to advance care redesign, practice transformation, and scalable value-based strategies.
  • Contribute to innovation initiatives that enhance MHN’s Medicare and Medicaid population performance.
  • Clinical Engagement & Partnership Development
  • Build and maintain strong relationships with practices, providers, and stakeholders to foster clinical integration and evidence-based care.
  • Lead provider engagement sessions to share best practices and address complex patient needs.
  • Serve as the clinical subject matter expert (SME) to the operations team, providing guidance on clinical workflows, protocols, and best practices to ensure alignment with organizational goals.
  • Partner with cross-functional teams to translate clinical insights into coordinated performance improvement.
  • Represent clinical priorities within cross-functional committees to advance MHN’s provider initiatives and objectives.
  • Clinical Informatics & Performance Improvement
  • Utilize data analytics and health information technology tools to interpret clinical data, identify performance trends, and generate actionable insights to inform care model improvement.
  • Serve as a clinical and FQHC subject matter expert to enhance technology integration and workflow optimization.
  • Define and track KPIs to measure quality, cost, and operational effectiveness.
  • Promote a culture of data-driven decision-making and continuous improvement.
  • Quality & Population Health
  • Support population health management by designing and implementing strategies that address the needs of the targeted patient population.
  • Collaborate on preventive care, chronic disease management, maternal care, and care coordination initiatives to improve population outcomes.
  • Monitor quality and outcomes data, contributing to annual Quality Improvement plans and performance reporting.
  • Partner with practices to drive adherence to evidence-based guidelines and enhance overall population health performance

WHAT YOU’LL NEED TO SUCCEED:

  • MD, DO, NP,PA or equivalent advanced clinical degree required.
  • Board certification in primary care specialty preferred.
  • Demonstrated success in clinical model innovation and care delivery redesign, translating strategy into practical implementation.
  • Demonstrated results in reducing cost of care and improving outcomes in managed care environments.
  • Deep understanding of value-based care, Medicare, ACO models, and Medicaid and safety-net populations, with the ability to apply this knowledge to improve outcomes and reduce total cost of care.
  • Expertise in FQHC operations, maternal care, and care delivery within community health settings.
  • Experience leading or co-leading committees, task forces, and cross-functional teams to align clinical, operational, and strategic objectives.
  • Exceptional relationship-building and stakeholder engagement skills across diverse provider settings.
  • Strong communication, presentation, and data-driven problem-solving abilities.
  • Ability to translate analytics and clinical insights into actionable strategies and operational execution.
  • Proficiency with EHR systems and clinical documentation
