THE OPPORTUNITY:

The EMR Optimization Lead will serve as a foundational leader at Medical Home Network (MHN). The EMR Optimization Lead is responsible for building and scaling our EMR optimization function to support value-based care across our network of partner organizations. This individual will play a critical role in advancing MHN’s mission to transform care in the safety net, improve health outcomes, and reduce disparities in underserved communities.

As the first hire in this function, this leader will not only drive EMR optimization strategy and execution but will also establish best practices, build strong partner relationships, and lay the groundwork for a high-performing team.

This role partners closely with community health centers, provider groups, and internal MHN teams to optimize EMR systems, enhance clinical workflows, and enable actionable, population-level insights that support value-based care initiatives.

    • Remote/Hybrid Schedule: This role follows a hybrid work model. Candidates within 50 miles of Chicago are expected to work onsite two days per week (Monday and Tuesday) and remotely three days per week. Candidates outside of the 50-mile radius are eligible for fully remote work.

WHAT YOU CAN LOOK FORWARD TO:

    EHR Optimization Strategy & Execution

    • Lead the development and execution of MHN’s EMR optimization strategy across partner organizations

    • Assess current EMR utilization and identify opportunities to improve workflows, quality performance, and operational efficiency

    • Partner with practices to enhance use of EMR systems in support of value-based care initiatives, including preventive care, chronic disease management, and care coordination

    • Drive adoption of standardized, patient-centered workflows aligned with MHN’s care model

    Client & Partner Engagement

    • Serve as a trusted advisor to clinical and operational leaders across MHN’s partner network

    • Build strong, long-term relationships through exceptional customer service, responsiveness, and subject matter expertise

    • Deliver training and education to stakeholders ranging from executive leadership to front-line staff

    • Effectively communicate complex EMR concepts in a clear, actionable way

    Data & Population Health Enablement

    • Ensure accurate and consistent capture of clinical and operational data within EMR systems

    • Partner with internal analytics and care teams to enable reporting on quality measures, risk adjustment, and utilization

    • Support data integration and exchange strategies to enhance population health management capabilities

    Leadership & Team Building

    • Serve as the leader and architect of MHN’s EMR optimization function

    • Establish processes, tools, and best practices that will scale as the team grows

    • Mentor future team members and contribute to hiring efforts as the function expands

    • Foster a culture of collaboration, accountability, and continuous improvement

    Innovation & Continuous Improvement

    • Stay current on EMR vendor capabilities and industry trends related to population health and value-based care

    • Identify opportunities to leverage technology to improve care delivery and reduce administrative burden

    • Contribute to broader organizational strategy by aligning EHR optimization with MHN’s clinical and operational goals

    • Experience in value-based care, population health, or working with safety-net providers (FQHCs preferred)

    • Prior experience in a lead or mentoring capacity, with the ability to build and develop teams

    • Experience with EMR integrations, data exchange (HL7, FHIR), or reporting tools

    • Strong project management skills with the ability to manage multiple initiatives simultaneously

    • Bachelor’s degree required (or equivalent relevant experience)

WHAT YOU’LL NEED TO SUCCEED:

    • 6+ years of experience in healthcare with a strong focus on EMR optimization, implementation, or clinical informatics

    • Proven experience working in a client-facing role, building and maintaining strong relationships with healthcare stakeholders

    • Hands-on experience with at least one leading ambulatory EMR (e.g., Epic, Athenahealth, eClinicalWorks, NextGen, Greenway)

    • Demonstrated success improving workflows, quality outcomes, or operational efficiency through EMR optimization

    • Strong communication skills with the ability to translate technical concepts into practical solutions

    • Experience in value-based care, population health, or working with safety-net providers (FQHCs preferred)

    • Prior experience in a lead or mentoring capacity, with the ability to build and develop teams

    • Experience with EMR integrations, data exchange (HL7, FHIR), or reporting tools

    • Strong project management skills with the ability to manage multiple initiatives simultaneously

    • Bachelor’s degree required (or equivalent relevant experience)
