THE OPPORTUNITY:

MHN is seeking a dedicated and compassionate Medical Assistant to support Annual Wellness Visits (AWVs) for Medicare patients in community health center settings near Tulsa, Oklahoma. This role offers a unique opportunity to advance preventive, geriatric-focused care for seniors in under-resourced communities.

In this position, you will help bring specialized geriatric expertise into community health centers that traditionally do not have dedicated geriatric services, improving both the level and quality of care delivered to older adults. The Medical Assistant plays a critical role in preparing patients for visits, supporting providers, and ensuring comprehensive wellness assessments that focus on prevention, early detection, and chronic condition management.

This is an onsite role working directly with patients and care teams to ensure high-quality, patient-centered care for aging populations. This role will work onsite at our partner health center locations across northeastern Oklahoma, including **Hulbert, Tahlequah, Muskogee, Pryor, Salina, and Westville.

    • This role will work onsite at our partner health center locations across Northeastern Oklahoma, including **Hulbert, Tahlequah, Muskogee, Pryor, Salina, and Westville.

WHAT YOU CAN LOOK FORWARD TO:

    • Prepare patients for Medicare Annual Wellness Visits, including intake, documentation, and vital signs.
    • Assist providers with geriatric-focused assessments, screenings, and preventive care services.
    • Collect and document patient medical history, medications, and health risk assessments in the electronic medical record (EMR).
    • Perform basic clinical tasks such as:
      • Vital signs
      • Height/weight measurements
      • Vision screenings
      • Fall risk assessments
      • Cognitive and depression screenings
      • Coordinate care activities and help ensure completion of recommended preventive services and screenings.
      • Support providers in identifying care gaps and opportunities for preventive interventions.
      • Educate patients on preventive care, wellness planning, and follow-up services.
      • Maintain accurate and timely documentation in compliance with clinical and regulatory standards.
      • Collaborate with physicians, nurse practitioners, care coordinators, and clinic staff to deliver a team-based care experience.
      • Ensure exam rooms and clinical supplies are stocked and prepared for patient visits.
