- $145,000-$160,000 annual base salary, dependent upon experience
- 15% target bonus, annual pay, based upon performance as assessed by management - bonuses will be achievable for all clinicians
- Serves as the "Quarterback" of the Care Team, providing longitudinal support of the patient care journey, developing individual care plans and assessing patient progress
- Provides expertise in general medicine and / or evidence-based behavioral health care to adult patients facing chronic and complex conditions (e.g., comorbid medical and mental health diagnoses, chronic medical conditions) both in the home and remotely, consulting with the broader internal team as needed for escalations
- Performs Initial Intake visits with patients, using assessment instrument criteria while engaging other interdisciplinary care team members, as required
- Orders, interprets, and analyzes diagnostic tests. Recommends follow-up appointments and further treatment options, as needed
- Prescribes and manages medications and treatments as appropriate including, but not limited to, the ability to execute injectables
- Captures diagnosis details, including billing codes, and submits claims
- Identifies social needs and non-medical barriers to care for the patient and collaborate with the Care Team to address
- Manages psychosocial component of each patient's care
- Actively engages with members of the interdisciplinary team via frequent patient rounding to drive efficient care planning
- Initiates internal referrals to engage other expertise within the Quartet Care Team
- Develop a strong rapport with a diverse set of patients (socioeconomic status, age, sexual orientation etc.) both in-person and via telehealth, supporting patient engagement in the program
- Uphold clinical customer service through the delivery of high quality, empathetic care
- Document concurrently all clinical findings and services provided in the electronic health record as required by compliance standards
- Active nurse practitioner license and certification. In good standing with the state of New Jersey Board of Nursing
- Expert in diagnosis and treatment of patients with chronic and urgent medical/psychological conditions
- Ability to travel locally at least 75% of the time to see patients in the home and community
- Have a valid driver’s license, active auto insurance, have access to a car and willingness to drive to patient homes/patient location
- Excellent written and verbal communication with an emphasis on clarity and compassion
- Strong computer skills, including experience working with Electronic Health Records (EHR).

- Maintain high levels of patient satisfaction
- Willingness to assist with after hours call coverage
- Experience in home-based services, complex care cases, or caring for patients with co-occurring substance use disorders
- Experience in value-based care models
- Previous telehealth experience
- Skilled at using online tools and technology to deliver care and communicate with patients
- Dual FNP / PMHNP background
- Highly collaborative and effective working in a team setting
- Bilingual candidate (Spanish and/or other languages)
- Comfort providing evidence-based care while being accountable for clinical outcomes and quality
- Thrive in a start-up environment setting the foundation for a new model of care
- Passionate about helping build innovative models of care that improve the lives of vulnerable populations
