About the Role:
We’re seeking an experienced Registered Nurse - Annual Wellness Visits (AWVs) to join our team.
In this consultant role, you will play a direct role in patient care by conducting comprehensive annual wellness visits via telehealth with physician supervision. You’ll support preventive care delivery, identify gaps in care, and collaborate with care teams to ensure patients receive a high-quality, personalized experience.
This is a flexible opportunity ideal for RNs seeking meaningful work with a reduced weekly commitment.
Responsibilities:
- Prepare charts prior to scheduled visits, verifying that all routine screenings, immunizations, and preventive services are current.
- Engage with Medicare patients to complete Annual Wellness Visits, including Health Risk Assessments, Cognitive Impairment Screenings, Personalized Prevention Plans, and more.
- Conduct telehealth visits (typically 20 minutes to 1 hour) to review paperwork, perform assessments, and ensure each patient’s record is accurate and complete.
- Work collaboratively with care teams to accomplish AWVs efficiently, supporting both patient satisfaction and operational goals.
- Follow established AWV protocols and standing orders, including delivering or coordinating any preventive services resulting from the visit.
- Act as a patient advocate by identifying and addressing care gaps, safety concerns, and social needs, and assisting patients in accessing appropriate resources.
- Communicate clearly with providers regarding any identified needs, risks, or follow-up requirements.
- Address open quality gaps with patients and work collaboratively to close them through education and follow-up.
- Accurately document all assessments, interventions, closed gaps, and patient interactions in the electronic health record (EHR), ensuring compliance with Medicare guidelines and internal policies.
- Participate in regular team meetings and quality improvement initiatives to support care delivery excellence.
- Maintain required annual competencies and stay current on best practices in preventive and geriatric care.
- Perform other related duties as assigned in support of Trovo Health’s mission and program goals.
Minimum Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the State of Minnesota.
- Prior experience conducting Medicare Annual Wellness Visits.
- Minimum 5 years of nursing experience in primary care, geriatrics, care coordination, or a related setting.
- Proficiency with electronic health records and technology-based care delivery.
Strong Additional Qualifications:
- Familiarity with telehealth and in-home care delivery.
- Strong organizational skills with the ability to manage scheduling and documentation independently.
- Excellent interpersonal and written communication skills.
- Comfort practicing candor and providing constructive feedback.
- Positive, patient-centered approach to care delivery.
