US Heart and Vascular (USHV) is seeking a full time Clinical Operations Nurse Manager – Value Based Care for the Dallas, TX area.
This new critical role supports our Value Based Care (VBC) clinical operations in north Texas overseeing our Integrated Care Clinics (ICC) for our complex cardiology patients and our Virtual Care Center that provide 24/7 triage, care management, and peri-procedural support. This role provides ongoing leadership for the development, implementation, and management of embedded complex clinical services for complex cardiology patients and the ongoing care management of a designated patient population. This role is crucial in leading our multidisciplinary teams in achieving optimal patient outcomes. In clinic and call center setting.
Responsibilities:
Under the direction and guidance of the VP of Value Based Care Operations:
- Leads and directs daily clinical operations inclusive of schedules, staffing, and patient care for assigned Integrated Care Clinics supporting in clinic complex patient care and the centralized Virtual Care Team supporting care management, triage, and peri-procedural support
- Develops strong, collaborative relationships with staff and physicians, fostering team-based care environment aligned with USHV VBC vision and strategies
- Develops and implements operational and clinical workflows to maximize operational efficiency and promote high quality patient outcomes aligned with USHV VBC performance expectations
- Collaborates closely with leadership on the development, implementation, and oversight of clinical protocols and standard workflows, clinical operations, and financial performance of clinic
- Ensures clinic patient panel and care management teams achieve population health targeted quality and outcome metrics
- Hires, trains, and supervises staff to ensure high caliber patient experience and optimal population health outcomes achieved
- Ensures patient and staff safety and infection control standards are met per policy standards, inclusive of following clinical policy and procedure, and maintaining compliance with all regulatory standards
- Ensures office and medical supply inventory and equipment is maintained and ordered as
Requirements:
- Excellent communication skills and organizational skills, with the ability to effectively interact with patients, healthcare providers, and internal teams
- Must be able to function independently within the limits of the position
- Ability to work collaboratively and cooperatively with an interdisciplinary team environment
- Ability to analyze processes and implement improvements to enhance patient outcomes and operational efficiency
- Excellent mentoring and coaching skills with ability to provide transparent and timely feedback to drive individual and team performance
- Experience in chronic condition management, care management, transitional care management, and ambulatory care setting
- Currently licensed to practice as registered nurse in the state of employment
- Current BLS for Healthcare Providers certification
Preferred:
- Bachelor’s degree or equivalent experience in business and/or health-related field
- 3 years or greater experience in clinic and/or population health operations management
- 1-2 years of experience in a clinical cardiology practice
